Build me a Claude Code plugin that sends smart desktop notifications when a task finishes, when Claude asks a question, when a plan is ready, when a review is done, and when there’s an API or session error.

I want it to work on macOS, Linux, and Windows, with a simple one command install and a setup command for changing sounds and settings. Notifications should be clickable on macOS and Linux so they bring the right terminal window back to the front, and it should handle common terminals and tmux or similar session setups. It would be nice if the notification title shows the current git branch too.

Please also include sound support, basic webhook options for tools like Slack, Discord, Telegram, ntfy, and custom webhooks, and make sure it feels lightweight, with no extra setup if possible. If you need current docs for any platform specific behavior, look them up online.
