Build me a simple web app for patient specific FMEA quality risk review in HDR GYN brachytherapy. I want the care team to be able to create an account, log in, open a patient workflow, and review each process step or sub step by entering or updating Occurrence, Detectability, and Severity, then have the app calculate and store the RPN for that patient.

Please make it feel practical for a clinic, with a clean main screen, easy navigation, and a way to save and print the risk review. It should help the team quickly see which parts of the workflow have the highest risk so they can focus on safety improvements. Use a new local database with sample data since the original database is not included. Also make the structure flexible so later we could add other treatment sites like breast, prostate, or head and neck by adding different workflow risk values.
