REIIdes/SuppliWise ? reverse-engineered prompt

Build me a full stack web app called SuppliWise that helps people figure out which supplements might be useful for them based on a health questionnaire. I want a clean, mobile friendly app where users can sign up, log in with email OTP, fill out a multi step assessment about their health, diet, symptoms, lifestyle, and goals, then get personalized supplement recommendations with explanations, confidence, and evidence sources. It should also have a dashboard where users can add supplements to a daily plan, mark them as taken, see streaks, adherence, and a calendar history.
