Build me a local Claude Code plugin for medical case review that helps a user turn a folder of patient documents into a structured second opinion packet.

It should read mixed case materials like reports, labs, discharge notes, pathology, and DICOM scans, then build a clear timeline, flag inconsistencies, and generate board minutes with consensus, dissent, confidence levels, missing data, and questions to ask the clinician. Keep the review text only, and make sure every claim comes with verified citations that were checked in session. I also want the imaging side to work deterministically, with DICOM to slice views and a simple 3D view from the scan data, but not have AI interpret pixels for the board discussion.

Make it feel like a real adversarial expert panel, with separate specialist roles, a devil’s advocate, a safety checker, and a cost conscious planner. Keep everything local, easy to inspect, and honest about not being a medical device or diagnostic tool. If you need to look up current docs online, do that.
