Build me a browser based clinical training simulator for medical students and newly graduated doctors. I want it to feel like an ER and a polyclinic mode, where I play the doctor, talk to AI patients in real time with my microphone, ask questions, order labs or imaging, diagnose, treat, and decide disposition. In the ER there should be multiple beds and tests should come back after simulated time passes. In the polyclinic it can be one patient at a time and tests can return right away.

After each case, give me an attending style evaluation that scores my communication, history taking, and clinical reasoning, and cites real guideline sources so the feedback is grounded. Keep the cases synthetic and make it clear this is for education, not real clinical advice.

Please make it work in the browser, with text chat as a fallback if voice is not running, and set it up so I can run the app locally and plug in my API keys on the server side. Look up current docs online if you need to.
