Build me a fresh Milpa app starter in PHP that works from the terminal, a simple text user interface, and MCP, all from the same declared operations.

I want the app to boot with a coa style command that discovers operations from plugins, shows them nicely, and runs them without me wiring each command by hand. Include a small plugin system, a few example operations for listing plugins, showing plugin details, validating a plugin manifest, and turning plugins on or off. Keep it file based, with no database and no web app framework.

Also add the shell and chat experience so I can browse available operations and talk to an agent that uses the same operations as tools. Make sure mutating actions are marked clearly and require confirmation or signing when needed, and that the rules are consistent across the terminal and MCP. If anything needs current package docs or best practices, look them up online and follow them.
