Build me a simple PHP Telegram bot using this iTelegram class.

I want a clean starter bot that connects with my bot token, reads incoming messages, and can reply to a few basic commands like /start and a fallback for anything unknown. On /start, send a friendly welcome message with the user name and a short note that the bot is running. If possible, include a small example of sending formatted text and replying in the same chat.

Also add a simple /update command that reloads the main class file from the repository link and confirms when the update is done.
