Build me a simple Telegram bot in JavaScript that I can run in Docker and control from chat.

I want it to feel like a small utility bot where I can send commands, get clear replies, and manage a basic call or telephony test workflow for my own lab or internal use. Keep the code tidy in one main bot file if possible, and make it easy to configure with environment variables. If anything depends on current library behavior, look up the latest docs online and use the best current approach.

Please make sure it starts cleanly, handles errors without crashing, and includes a few useful commands so I can test that the bot is alive and responding. If there are places where the app would need safety checks or limits, add them in.
