Build me an open source AI medical scribe app that lets a clinician record a patient encounter, transcribe the audio, and turn it into a clean structured draft note automatically.

I want it to feel simple for a doctor or nurse to use, with a quick setup, a clear recorder screen, a note preview, and easy editing before saving or copying the final note. It should keep privacy in mind, support local transcription, and let me run it without being locked into one vendor. If possible, make the default setup work with local Whisper for transcription and a hosted AI model for note generation, but also include a fully local desktop option for people who want everything on their own machine.

Make it work as a web app and also as a desktop app if that fits. Use current docs online if you need to.
